Forms & documents
Waivers, health declarations and other paperwork for exhibitors and competitors. For entry closing dates and fees, see How to enter.
Waivers
Complete a waiver if you are competing in horse events, handling animals, or taking part in physical events such as the rodeo, woodchop, Young Farmers Challenge or hay stacking. Everyone entering the McGoldrick Main Arena to compete must sign one; forms are also available at the Announcer’s Box.
Health declarations
- Cattle: National Cattle Health Declaration (fillable PDF)
- Goats: National Goat Health Declaration (fillable PDF)
Read our biosecurity rules before you bring animals to the Show, including the electronic tag (NLIS eID) requirement that starts on 1 January 2027.
Property Identification Code (PIC)
In NSW, owners of livestock such as cattle, sheep, goats and pigs need a Property Identification Code when trading or moving their animals. A PIC is an eight-character number issued by Local Land Services, and it lets livestock be traced to help manage disease and chemical residues. Apply for a PIC with Local Land Services.
The Berry Showground’s PIC is NK464606.
